Job Description

Maxxima is currently seeking an experienced QUALIFIED SOCIAL WORKER to fill a full time role in LANCASHIRE. The ideal candidate will have a significant understanding of ADULTS Services.

The starting pay rate for this locum QUALIFIED SOCIAL WORKER job is £21 PER HOUR and is initially a 3 month contract with potential for extension.

Requirements of the role

All applicants must have a relevant qualification in Social Work (e.g. DipSW), be registered with the HCPC and have a minimum of 2 years' post-qualifying experience in a similar statutory team.

To accept referrals of members of the public and/or their carers who require assessment of their needs and carry out the assessment and take appropriate action including providing a social work service. Within this context Social Workers are expected to service the whole of the multi-racial community in Lancashire.

The post holder is expected to carry out their duties and responsibilities in accordance with the County Council's Policies and Procedures and the Directorate's Statement of Principles and Standards of Conduct.

To instil, promote and maintain a health and safety environment with all levels of Management, Employees and Service Users within the service as directed by legislation and the County Council's policies.

A core function of the officers of the County Council is to deliver and promote the positive benefits of equality and diversity in the way they carry out their duties and responsibilities.
To ensure that the Social Care needs of individuals in the community or in hospital, are identified and met within the requirements of legislation and available resources.
To co-ordinate a comprehensive holistic assessment of need, identify risk and work with the multidisciplinary team to manage risk. Establish, monitor and review a care plan that is agreed with the service user, carer(s) and service providers.
To ensure the provision of high quality services to individuals through creative, cost effective care planning using the self-directed support process which identifies measurable outcomes for service users and carers.
