Maxxima is currently seeking experienced QUALIFIED SOCIAL WORKERS to fill full time roles in HAMPSHIRE. The ideal candidate will have a significant understanding of ADULTS Services.
The salary for these locum Social Care positions is £30 Per Hour (Umbrella Rate) and is initially a 3 month contract with potential for extension.
Requirements of the role
We are looking for candidates who have experience of working within a busy Adults statutory service
To primarily work within the Hampshire County Councils Continuing Healthcare (CHC) team in the assessment of eligibility for CHC.
To use professional knowledge, skills and interventions in accordance with HCPC code of practice, statutory duties, professional practice, departmental
policies and procedures, and best value. To assess, monitor and review the social care needs of adults. To support the planning and arrangements, provision or purchase of appropriate services/care that will meet a person's assessed and eligible needs and the outcomes identified. To work in partnership with users, carers, relatives, health and other agencies, in engaging in an individuals CHC assessment and MDT recommendation. The post will be required to work across Hampshire so can be flexibly based, so a car driver is essential. They will involve networking between health and social care and travelling to different localities in Hampshire.
An understanding of the national Framework for Continuing healthcare (revised 2018) is essential as is experience of support and engaging in the continuing healthcare process.
