Maxxima is currently seeking an experienced QUALIFIED SOCIAL WORKER to fill a full time role in NORTHAMPTONSHIRE. The ideal candidate will have a significant understanding of ADULTS MENTAL HEALTH Services.
The starting pay rate for this locum QUALIFIED SOCIAL WORKER job is £23 PER HOUR and is initially a 3 month contract with potential for extension.
Requirements of the role
All applicants must have a relevant qualification in Social Work (e.g. DipSW), be registered with the HCPC and have a minimum of 2 years' post-qualifying experience in a similar statutory team.
Band 6 Mental Health Social Worker Required
Dementia and Delirium team, Northampton and Kettering.
An opportunity has arisen for a self-motivated and resourceful Senior Mental Health or Dementia Specialist to join our busy team.
Based within Northampton or Kettering General Hospital along with some community working as a mental health professional, you will be responsible for the assessment and treatment of Older Adult patients who are admitted onto the dementia and delirium pathway. The D&D service specialises in assisting with complex discharge planning for people with dementia and/or delirium who are acute hospital patients. The D&D model works closely with inpatient and community services in order to support patients and their families with the most appropriate discharge plan helping reduce delayed discharges and giving the patient the best chance of returning home.
The role will include completing triage screening and assessments of older people who have been referred to the D&D team with complex needs as well as advising on treatment plans, undertaking risk assessments and assisting with planning packages of care. There will be a need for MDT and collaborative work with a range of other providers. The post holder will provide mental health interventions to individuals as appropriate and provide an expert advisory role to colleagues in the General Hospital. The post holder will be involved in the on-going development of the Dementia and Delirium service for older people within the acute general hospital.
You will be supported in that role by a Band 6 occupational therapist and a Band 4 Rehabilitation Assistant.
The successful candidate must have excellent communication and assessment skills, be confident in their ability to support service users with complex mental illnesses, and most importantly be a team player but also able to work autonomously.
