Job Description

Maxxima is currently seeking experienced CARE MANAGEMENT OFFICERS to fill full time roles in LEICESTER. The ideal candidate will have a significant understanding of ADULT Practices.

The starting pay rate for these locum Social Care positions is £13 Per Hour and is initially a 3 month contract with potential for extension.

Main Objectives:
Receive initial contacts and referral for adults social care.
Screen initial contacts, make preliminary identification of need and match assessment response to this.
Provide an effective response to emergency situations.
Undertake assessments and arrange services to meet identified needs in line with the Department's eligibility criteria. i.e. FACS
Co-ordinate and arrange various community care services and advise and explain financial implications.
Monitor and review community care services. To manage risk as appropriate.
Work with other agencies from the Voluntary Sector, PCTs and other health colleagues to provide appropriate short term support.
Be proactive in implementing LCC Equal Opportunities policies and practices.
Ensure provision of services appropriate to the needs of a multi-racial, multi-cultural and multi-lingual population.
Be committed to the LCC's Eco Management Audit System (EMAS).

Summary of Job Tasks:
To take part in the provision of a duty service.
To screen new all referrals; giving appropriate advice, information and assistance when necessary.
To liaise with other Social Care staff and with professionals from other agencies.
To undertake assessment of service users' needs for services.
To undertake short term work with service users in primary care settings and prevent deterioration of social functioning.
To arrange for appropriate services to be provided.
To identify when specialist involvement is required.
To monitor and review service provision.
To keep written and computer records on work undertaken.
To work within, and contribute to, the development of Departmental, divisional and team policies and procedures.

Essential Job Requirements:
NVQ3 or equivalent (or equivalent experience in care services)
Substantial relevant employment experience involving provision of social care services
Experience of maintaining effective and confidential records
