Job Description

Maxxima's Nursing team are currently recruiting for a Band 7 Discharge Nurse Assessor to fill a job role Chelmsford, Essex. The ideal candidate will have a significant understanding of complex discharging practices within the NHS.

This NHS role will require you to identify and manage patients whose discharge needs are complex and fall outside the criteria of simple discharges. You will undertake Continuing Healthcare (CHC) assessments and create plans for patients with these needs.

The pay rate or this locum Band 7 Discharge Nurse Assessor job is £28.00 per hour (Umbrella) and is a full-time, ongoing contract due to start asap.

Requirements and details of this Band 7 Discharge Nurse Assessor vacancy:

*You must be a registered general nurse with a valid NMC pin
*You must have previous experience as a discharge coordinator or assessor
*You must have previous experience within Continuing Healthcare assessment (CHC)
*You must be able to work independently and as part of a team
