Job Description

Maxxima is currently seeking an experienced Band 7 Complex Case Coordinator (RMN/RNLD) to fill a full time role based in Lancashire. This is an exciting new opportunity, involving visiting nursing homes/care homes and completing at least 7 assessments/reviews per week, presenting to panels as required.

This is a full time role (Monday-Friday), and the rate of pay will be £27.00 per hour (umbrella rate). This is initially a 2 month contract, to start ASAP.

The role will also include:

*To review/assess packages of care for patient's subject to section 117 aftercare
*To identify individual ongoing health care needs and coordinate the process of identifying and securing appropriate commissioned services, either through CHC or joint funding arrangements with the relevant Local Authority
*To manage / co-ordinate NHS Funded Nursing care on behalf of CCGs
*To foster effective working relationships and processes with partner individuals/agencies e.g. NHS Trusts, General Practitioners, Primary Health Care Teams, Local Authority, Independent Providers and Voluntary Services
*Responsible for ensuring effective assessment, and review for clients in secure commissioning placements, or subject to section 117 aftercare
*Advise and support CSU operational teams with regard to their responsibilities concerning clients with mental health needs
*Support the provision of clinical supervision to RMNs in the CSU
*Support the new Five Year Forward View Mental Health agenda and be flexible to ensure that effective transformation is supported from a clinical commissioner perspective.
*Review existing care provision and ensure it is still appropriate, if not alternative care pathways will need to be sourced and recommended in line with partner agencies
*Arrange (with CSU Commissioning colleagues), monitor and review individual complex packages of care for patients with complex physical disabilities and or Mental Health with multiple providers
*Input appropriate data onto the CSU database and ensure all communications, decisions and actions are recorded within paper based client files and are stored electronically.
