Maxxima is currently seeking experienced Band 6 Learning Disabilities Nurse or Qualified Social Worker to fill 2 full time roles in Hull. The ideal candidate will have a significant understanding of working with Adults with Learning Disabilities in the community.
The starting pay rate for this locum Band 6 Learning Disabilities Nurse /Social Worker role is £22.00 per hour (umbrella) and is initially a 3 month contract with potential for extension.
The community learning disability team (CTLD) is the first point of contact to both inpatient and community services. Individuals accessing the service have a learning disability diagnosis, are over 18 years of age or in transition from children's services and require a period of joint working to be undertaken prior to transfer.
You will be working as part of a multi-disciplinary team, within the community, to provide services such as further specialist assessments, such as tests of cognitive functioning, person-led assessments to determine levels of care required within support plan, personalised services which support the individual at home, psychiatric assessments and providing transitioning support for young people transferring into adult services
Requirements of the role
- All applicants must have a relevant qualification in Social Work (e.g. DipSW) or be a dual qualified Nurse and Social Worker ,be registered with the HCPC or NMC and have a minimum of 2 years' post-qualifying experience in a similar team.
As this is a community role, you will need to be a car driver with access to a vehicle
- You must be able to commit to at least 3 months, and be able to start within June 2018.
