Job Description

Maxxima Nursing Team is currently seeking an experienced Band 6 CHC Nurse Assessor to fill a full time role in Medway. The pay rate for this locum Band 6 CHC Nurse Assessor job is up to £30.00 per hour (Umbrella or LTD) and this role falls outside IR35. This role is initially a 2 month contract, with the likeliness that it will extended to 6 months, due to start in April 2018.

You will be required to contribute to the delivery of highly effective adult services in an efficient manner, the service must be delivered using a strengths-based, personalised approach. Your duty will be to work within organisational policy and legislative requirements including The Care Act, Mental Capacity Act and Mental Health Act and you must promote support the Council's policies and procedures for safeguarding.

The successful candidate will helping the council to identify who is eligible for continuing health care funding by completing CHC checklists, then progressing cases through DST within the 28 day timescales.

Requirements of the Band 6 CHC Nurse Assessor role:

- You must be a Registered General Nurse with a current NMC pin
- You must have at least 5 years post registration experience
- You must have previous experience of completing DST's, Health Needs Assessments and FNC assessments for patients out in the community
- You must have 2 years' experience in implementing the National Framework for Continuing Healthcare
- You must have a licence and access to your own vehicle
