Maxxima is currently seeking an experienced Band 6 Care Coordinator to fill a full time, 9-5 Monday to Friday, role based in Rotherham. The ideal candidate will have a significant understanding of Mental Health practices.
The starting pay rate for this locum role is £23 per hour (Umbrella Rate) and is 3 months initially with the possibility of extension to start ASAP.
The service triages and assesses all referrals to identify the most appropriate service to meet individual need when persons are experiencing mental health issues and complete a pre-determined number of allocated assessments per week with the outcome of signposting service users to the most relevant service. The team currently consists of Community Mental Health Nurses, Approved Mental Health Professionals, Consultant Psychiatrist and other associated medical staff.
*To provide a clinical service that is service user focussed.
*To triage all mental health referrals to the SPA Team to determine the priority for service response and the appropriate clinical pathway.
*To carry out a health and social care assessments including comprehensive risk assessments of persons referred or self -referred to the service.
*To provide brief interventions to service users where required
*To recommend and refer, where treatment is indicated, to the most appropriate team/service utilising all available resources.
*To identify where appropriate treatment based interventions for all service users ensuring that best practice is implemented.
*Where appropriate to participate in Mental Health Act Assessments so that alternatives to formal hospital admission are considered.
*To respect the views of service users, carers, families and significant others, and work collaboratively with them when planning care and assisting them to meet their needs.
Requirements of the role:
