Maxxima is currently seeking an experienced Band 6 Care Coordinator/RMN to fill a full time, 9-5 Monday to Friday, role based in Rotherham. The ideal candidate will have a significant understanding of Mental Health practices.
The starting pay rate for this locum role is £23 per hour (Umbrella Rate) and is 3 months initially with the possibility of extension to start ASAP.
We are a specialist service who strive to deliver excellent care, therapies and interventions to individuals and their carers who are trying to work their way out of a severe and disabling mental health problem. Some of these problems may include personality disorder, severe anxiety, severe depression, obsessive compulsive disorder, eating disorders, phobias and other difficulties that are likely to be caused by past life experiences which may include trauma. We are a dedicated and highly skilled team trained in a range of therapies, social work and other evidence based interventions to help guide you towards your own personal goals for recovery.
Main Tasks of the Role:
*To assess Team referrals, plan, implement and evaluate care within a clinical caseload utilising a collaborative, patient focussed approach.
*To be responsible for maintaining a systematic appointment system.
*To liaise with GP's/Primary Care and all relevant services across the Trust.
*To work closely with the relevant Pharmacy services.
*To maintain accurate, contemporaneous, accessible records according to professional and Trust record keeping standards.
*To manage a clinical caseload acting as care co-ordinator, lead professional and group facilitator within the Intensive Community Therapies Team.
Requirements of the role:
All applicants must have an updated NMC Registration.
All applicants must have experince within the NHS.
All applicants must have experince working within the community.
All applicants must be a car driver.
